Do I need a physician referral or prescription to be seen?No. In the state of Ohio, you do not need a physician's referral or prescription to be seen by a physical therapist. Physical therapists in Ohio have "direct-access" privileges which allow them to see patients without a physician referral or prescription. Your insurance company may require a physician's referral or prescription if you plan to submit for reimbursement.

Why cash-based physical therapy?We're here for you, not insurance companies! We are not bound by insurance restrictions. Insurances can restrict the number of treatment visits, the frequency of treatment visits, the treatments and interventions used, etc. Waiting for insurance approvals can delay your care. Being a cash-based practice allows us to provide the best possible care and do what's best for you. Because we are cash-based we can: *Provide individualized 1-on-1 care. *Provide the most appropriate treatments and interventions required for your condition. *Provide treatments and interventions not covered by insurance. *Provide the necessary visits and treatment time per session. *Provide timely care without waiting for insurance approvals.

What is the cost for services?Call us! We would love to work with you! You'll likely find that our prices our significantly less than an insurance-based local hospital or institution. This may make a significant difference if you are paying out of pocket, have not met your insurance deductible, have a high copay, or your insurance requires you to pay a percentage of services. If you plan to self submit for insurance reimbursement, you should contact your insurance company before starting therapy to check on reimbursement for out-of-network physical therapy and any requirements for reimbursement including preauthorization. Please note that Medicare does not reimburse clients for services provided by a practitioner that is not enrolled with Medicare. Clients can not self submit for reimbursement by Medicare if a practitioner is not enrolled with Medicare. Campbell Physical Therapy and Wellness is not enrolled with Medicare.

Do you take my insurance?In order to provide the highest quality care without the limitations and restrictions of insurance companies, Campbell Physical Therapy and Wellness is a cash-based or self pay practice and does not take insurance. Our practice is considered an "out-of-network" provider and does not submit insurance claims. Some patients submit a paid bill for services to their insurance for possible reimbursement. Depending on your insurance plan, deductibles, copays, etc. you may actually save money at Campbell Physical Therapy and Wellness versus an insurance based clinic. When contacting physical therapy clinics, ask for their rates and then compare them to Campbell Physical Therapy and Wellness. If they refuse to give you their rates, think twice about using their services. Please note that Medicare does not reimburse clients for services provided by a practitioner that is not enrolled with Medicare. Clients can not self submit for reimbursement by Medicare if a practitioner is not enrolled with Medicare. Campbell Physical Therapy and Wellness is not enrolled with Medicare. At Campbell Physical Therapy and Wellness you may pay for services via credit card, HSA, FSA, cash, or check.

Can I get reimbursed by my insurance?Clients can submit self-claims to insurance companies for possible reimbursement. Campbell Physical Therapy and Wellness will provide you with the necessary receipts and codes for you to submit your claim. The amount of reimbursement or payment toward your deductible is entirely dependent on your insurance company and specific insurance plan.
